I wanted to follow up and give you an update on Madison. The new medication you prescibed was working well along with 1 capful of miralax then Madison started to become constipated again about a month ago. We then increased to 2 capfuls of miralax daily. She is now back down to a 1/4 cap of miralax daily and her stool is like water. This has been a roller coaster the month of August. She continues to go between constipated and water.
Do you have any recommendations? Also, we never received a call from your office regarding the breath test or the upper gi test. Is this something that you still would like Madison to have done?

Decision regarding escalation to invasive testing
High‑risk medication management
Examples in GI that can qualify if documented correctly:
Considering enteral feeding
Considering TPN
Considering repeat EGD in high‑risk patient
Severe medication intolerance with limited options
Failure of multiple neuromodulators → escalation discussion
Key phrasing:
“Given refractory symptoms and medication intolerance, there is increased risk of clinical deterioration.”
